Replacing a Lost DD Form 214, "Certificate of Release or Discharge"
Or, you can request the DD Form 214 by mail by sending a SF 180, "Request Pertaining to
Military Records" or letter to the National Personnel Records Center. Include the following
information in your letter:
- Your full name
- Social Security number
- Current phone number (including area code)
- Approximate dates of service
- Place of discharge
- Return address
- Reason for request
